Requirements and Procedures:
A. Regular Program
For High School Graduates
 - Application form with two pcs of 2x2 picture
 - Application fee of P150
 - Photocopy of high school report card
 - Photocopy of high school diploma
 - Photocopy of Certificate of Good Moral Character
 - Photocopyof Birth Certificate
 - 1 clearance (either of the ff: NBI, police, baranggay clearance)
 - Interview with School Director
 - Medical Exam (urinalysis, fecalysis and chest x-ray)
For Transferees
 - Application form with two pcs of 2x2 picture
 - Application fee of P150
 - Photocopy of Birth Certificate
 - Photocopy of honorable dismissal
 - Photocopy of Transcript of Records
 - 1 clearance (either of the ff: NBI, police, baranggay clearance)
 - Interview with School Director
 - Medical Exam (urinalysis, fecalysis and chest x-ray)
B. Short Courses
 - 1 pc. 2x2 / 1x1 picture
 - 2 resumes in separate folders
 - 1 clearance (either of the ff: NBI, police, baranggay clearance)
 - medical exam (urinalysis, fecalysis, chest x-ray)
 - waiver
 - evaluation form
 - time card
C. On-the-job training
 - recommendation from the school/manpower agency
 - 2 resumes in separate folders
 - 1 clearance (either of the ff: NBI, police, baranggay clearance)
 - medical exam (urinalysis, fecalysis, chest x-ray)
 - waiver
 - evaluation form
 - time card

PAYMENT PLAN
  
Plan A
  • Full program course (COC, Certificate Program and Diploma Course)                
  • Full payment with 5% Discount                             
  • Refund Policy will be the same as Plan B
Plan B
  • Full program course (COC, Certificate Program and Diploma Course)
  • Reservation Fee of Php10,000 – payment of which may be deducted to internship fee and the last module before internship and is non-refundable
  • 50% of the tuition fee per module – payable before start of class per module
  • 50% of the tuition fee per module – payable before midterm exam per module 
Plan C
  • Modular with option to convert to regular course
  • Shot course fee – once the student decides to convert to regular program, provided done within the same academic year he started, all payments made shall be converted / considered as payment based on Plan B.
  • The student cannot anymore convert if he /she already receives a CHAMP Short Course Certificate.  as an option, he may return the same certificate when he decides to convert to regular program.
  • The CHAMP Short Course Certificate is not the same as the certification given to student  Under a regular program when he/she finishes a module 
Plan D
  • Short Courses Only- 50% of the Short Course fee per module – payable before start of class per module
  • 50% of the Short Course per module – payable before midterm exam per module
